In today's healthcare landscape, there's a huge need for someone who can do it all - answer medical questions, navigate health coverage, and be your patient's biggest supporter.
That's where you come in.
Our patient care navigators fill the gaps in a patient's care team. We're excited to offer this course and train you to help one of the most vulnerable populations in the United States.
This four-part educational series will equip you with the critical competencies to work with us as a patient care navigator at Leadership Health.
Who we are
Leadership Health was founded in 2011 by the largest private medical group in the state of Maryland with the idea that telehealth, house calls, and remote patient monitoring can be utilized to provide cutting-edge case management for individuals at high risk of hospitalization.
Our case management system provides individuals with support services – not to replace their doctors or existing care team, but to provide high-level oversight to ensure risk factors are monitored, their questions are answered, and that they have everything they need to get on the path to better health.
What will be covered in this course:
Insurance
Medical insurance can be a maze to navigate. Thankfully, your patients have you as a resource to answer their questions and break down their options. You'll learn the answers to common questions about Medicare and other terminology, so that you can explain to them their coverage.
Medical
When do patients see a Specialist instead of a Primary Care Physician? What kinds of conditions are you likely to encounter? How do you analyze an Encounter Note? Learn the basics to prepare for any and all medical questions your patient might ask you.
Support
With your professionalism, exceptional bedside manner, and techniques incorporated from Motivational Interviewing, you'll help your patients feel supported as they embark on their journey to better health.
